Dr. Hussar Boyle does not accept insurance. Her practice is private pay only. Superbills that the client can submit to their insurance company for potential Out of Network (OON) reimbursement can be provided upon request. We highly suggest you contact your insurance provider directly to learn more about your OON benefits. However, please note insurance companies do not typically reimburse for psycho-educational assessments.

More About Evaluations & Fees
Fees for all evaluations:
$100 of the total fee is due at the intake appointment.
Payment for 50% of the remaining total is expected on the first day of testing.
The remaining balance is expected at the assessment feedback session.
Alternative payment plans are available based on client need.
Flat Fees are based on the focus and extent of the evaluation required to answer the referral question(s). This is discussed in depth and agreed upon at the intake session.
All evaluations include:
an intake session with parents/guardians (approximately 1 hour).
all necessary testing sessions.
a feedback session with parents/guardians to review results and discuss recommendations (approximately 90 minutes).
a written evaluation report is provided within 4 weeks of the feedback session appointment.
A detailed statement of fees (called a superbill) will be provided upon request for clients to submit to their insurance companies for self-filing claims. This will allow you to seek potential reimbursement using your out of network (OON) benefits.

More About Insurance
Why don't I take insurance? Insurance companies only pay for services that they consider ‘medically necessary.’ This means that most insurance companies do not cover psychological testing for developmental delays or learning issues, as they deem these services ‘educational’ rather than ‘medical’, even if the provider is ‘in-network’. In addition your insurance company will state: “A quote for benefits does not guarantee payment…” This means that you can be told that a service is covered and given an authorization number. Yet, the claim is often still denied once the insurance company reviews the diagnosis. Meaning you are responsible to pay for the evaluation out of pocket anyway.
Another issue is that pre-approvals/pre-authorizations involve the staff of the insurance company ‘granting permission’ for each and every individual test that I professionally deem necessary to administer, prior to starting testing. I tailor my assessments to the client and the referral question, so I frequently change the test battery after I start working with- and getting to know- the client, rather than administering the same tests to every client I work with. This individualized approach does not fit with the insurance pre-approval/pre-authorization process.
Lastly, I am required to give a mental health diagnosis/code in order to receive reimbursement for services through the client's insurance. This is unethical if in fact the client does not meet criteria for a diagnosis, and this also requires that the client's diagnosis be shared with third parties.
Evaluation Types and Associated Fees
Comprehensive Diagnostic Psychological Evaluation
Fee: $2,000 Approximate number of hours of overall assessment: 20 hours
A comprehensive diagnostic psychological evaluation explores how cognitive, attention, and emotional/behavioral factors may be effecting academic, social, or functional areas. This is the most comprehensive evaluation I offer, as it covers all possible areas of difficulty, including attention, learning, emotional/behavioral, and other disorders. Common disorders assessed for by this evaluation are Learning Disability (Reading, Math, Writing), Intellectual Disability, ADHD, Autism, and psychological disorders (such as anxiety and depression). This evaluation is typically requested/recommended when a client's case is highly complex (for example medically), when the client has never been evaluated previously, or when a second opinion is necessary.

academic achievement assessment in all academic areas and/or curriculum based measures (CBM)
behavior/emotional/social assessment as needed
assessment of adaptive/daily living skills as needed
testing of all neuropsychological processing areas appropriate, such as processing speed, visual-motor skills, visual-perceptual skills, auditory processing skills, phonological processing skills, attention/concentration skills, oral language, retrieval speed

Diagnostic Clarification Psychological Evaluation
Fee: $1,500 Approximate number of hours of overall assessment: 15 hours
The purpose of this evaluation is to add onto a previous evaluation or perform a reevaluation to update information regarding current levels of functioning. It is similar to the Comprehensive Psychological Evaluation, however the evaluation includes fewer components and is not as time consuming or as extensive. This evaluation is typically recommended for clients who have already had an initial and complete psychological evaluation previously (for example by their school district) and require diagnostic clarification or whose previous psychological evaluations did not assess for all areas of current concern.

academic achievement assessment in select academic areas and/or curriculum based measures (CBM)

Developmental/Behavioral Psychological Evaluation
Fee: $1,000 Approximate number of hours of overall assessment: 10 hours
This evaluation is most appropriate to facilitate accurate identification of neurodevelopmental disorders such as Autism, Global Developmental Delays, Intellectual Disabilities, ADHD, and functional difficulties associated with birth injuries as well as genetic or metabolic disorders. Further this evaluation is recommended for clients who have been previously evaluated (for example by their school district) and have up to date evaluation information. Clients who have never been evaluated previously will require assessment through the Comprehensive Diagnostic Psychological Evaluation option. In addition clients who have been previously evaluated but are seeking diagnostic clarification or whose previous evaluations did not assess for all areas of current concern will be be evaluated through the Diagnostic Clarification Psychological Evaluation option.

The purpose of this evaluation is to determine gifted and talented abilities and functioning.
This evaluation typically includes:

comprehensive academic achievement testing in all academic areas (basic reading, reading comprehension, reading fluency, math calculation, math reasoning, math fluency, written expression, spelling)

Mental Health Psychological Evaluation
Fee: $600 (excluding cognitive and/or achievement assessment) Approximate number of hours of overall assessment: 6 hours

This evaluation assesses how an individual’s mental health impacts overall functioning and relationships. This evaluation focuses on the individual’s behavioral style and emotional development. Based upon presenting concerns, cognitive and/or achievement testing may or may not be included. Common psychological/mental health disorders evaluated for with this evaluation are anxiety and depression.
This evaluation typically includes:
cognitive/intelligence assessment in select areas as needed
academic achievement assessment in select academic areas and/or curriculum based measures (CBM) as needed
behavior/emotional/social assessment

Cognitive/Intellectual Evaluation

The purpose of this evaluation is to update or reevaluate cognitive functioning. A cognitive test (commonly known as an IQ test) is administered as the only component of this evaluation (sometimes a measure of adaptive skills is also necessary and is also included in this evaluation). This type of evaluation may be requested by a government agency/program, a college, or a private mental health or agency program.

brief written report of performance with score report
Academic Achievement Evaluation
Fee: $400 Approximate number of hours of overall assessment: 4 hours
The purpose of this evaluation is to update or reevaluate academic achievement functioning. An achievement test is administered as the only component of this evaluation. This type of evaluation may be requested by a government agency/program, a college, or private mental health or agency program.
